Can Apple Watch Detect Sleep Apnea?

Apple Watch can flag patterns linked to moderate-to-severe sleep apnea, but it cannot diagnose the condition. Learn how the feature works and what to do next.

Yes, with an important limitation. A compatible Apple Watch can issue an FDA-cleared notification when wrist-motion data collected over multiple nights shows patterns associated with moderate-to-severe sleep apnea. It cannot diagnose sleep apnea, count your apneas, determine their cause, or rule out the condition when no notification appears 1.

The most common form, obstructive sleep apnea, occurs when the upper airway repeatedly narrows or closes during sleep. Central sleep apnea involves a problem with the signals that control breathing. An Apple Watch notification does not distinguish between these conditions, so a clinician still needs to evaluate the result and choose an appropriate sleep test 2 3.

What the Apple Watch actually measures

The Sleep Apnea Notification Feature uses accelerometer signals to identify fine wrist movements associated with temporary changes in breathing. It does not listen for snoring or rely on the Blood Oxygen app. The software converts the motion data into a nightly Breathing Disturbances estimate 4 1.

The Health app may classify a night as:

  • Elevated: the watch identified an elevated number of breathing disturbances per hour.

  • Not Elevated: the watch did not identify an elevated number that night.

  • No Data: the watch could not calculate a result, which can happen if it was not worn, could not collect a usable signal, detected excessive motion, or restarted during sleep 5.

Breathing Disturbances are not the same as the apnea-hypopnea index, or AHI, used in sleep testing. You should not compare the two values one for one. AHI is calculated from a diagnostic recording that measures airflow and other physiologic signals; the watch estimates a different metric from wrist motion 5 3.

Why the feature needs several nights

The notification algorithm looks for a repeated pattern rather than judging one unusual night. It works in consecutive 30-day windows and needs at least 10 nights with Breathing Disturbances data before it can produce a possible sleep apnea notification. It continues checking in later 30-day windows as more data becomes available 5.

A single elevated night does not mean you have sleep apnea. Apple notes that sleep position, alcohol, an upper respiratory illness, and some medicines can raise Breathing Disturbances on a particular night. Looking across several nights reduces the influence of an isolated change, but it does not turn the result into a diagnosis 5.

Which Apple Watch models support the feature?

Apple currently lists Sleep Apnea Notifications for Apple Watch Series 9 and later, Apple Watch Ultra 2 and later, and Apple Watch SE 3. You also need a paired iPhone, current iOS and watchOS software, sleep tracking enabled, and access to the feature in your country or region 6 7 8.

The feature is intended for adults age 18 or older who have not previously received a sleep apnea diagnosis. It is not intended to monitor known sleep apnea, measure whether treatment is working, or guide changes to CPAP or another therapy 1 5.

If you already have sleep apnea, use the follow-up plan and treatment data recommended by your clinician. Do not change treatment because the watch shows an elevated night, a normal-looking trend, or no notification.

How to turn on Sleep Apnea Notifications

Before setup, charge the watch enough to track sleep and wear it snugly on top of the wrist. Then:

  1. Open the Health app on your iPhone.

  2. Tap Search, then Respiratory.

  3. Under Sleep Apnea Notifications, tap Set Up.

  4. Confirm your date of birth and whether you have been diagnosed with sleep apnea.

  5. Follow the remaining prompts and make sure Track Sleep with Apple Watch is turned on.

Wear the watch consistently during sleep. Ten usable nights are the minimum for an evaluation window, but wearing it more often gives the algorithm more opportunities to collect data 6 5.

You can review Breathing Disturbances in the Respiratory section of the Health app. If you receive a possible sleep apnea notification, the app also lets you export a PDF report to share with a healthcare professional 6.

How accurate are the notifications?

The most useful accuracy figures come from Apple's prospective clinical validation submitted to the FDA. The study enrolled 1,499 adults who wore an Apple Watch for up to 30 nights and completed at least two nights of home sleep apnea testing. Of those participants, 1,278 had enough watch and reference data for the notification analysis 1.

For identifying participants whose home tests showed moderate-to-severe sleep apnea:

  • Sensitivity was 66.3%. In this study, the feature notified about two out of every three participants in the target group. About one-third were not notified.

  • Specificity was 98.5%. Among participants whose reference tests fell in the normal-to-mild range, the feature correctly remained silent in the large majority.

Sensitivity also differed by severity. It was 43.4% in the moderate subgroup and 89.1% in the severe subgroup. The algorithm was deliberately designed to favor high specificity, which reduces false alerts but also means it misses some people who have the condition 4.

These percentages describe performance in a study population, not the probability that one person's result is correct. The reference testing used a specific 4% oxygen-desaturation rule for scoring hypopneas, and the feature targets patterns associated with moderate-to-severe disease rather than every case of sleep apnea 1 4.

The practical interpretation is:

  • A notification is a credible reason to arrange a clinical evaluation, but it is not proof of sleep apnea.

  • No notification is not reassurance that you are free of sleep apnea, especially if you have symptoms.

  • Nightly Breathing Disturbances cannot tell you your diagnostic AHI or disease severity.

What to do after a possible sleep apnea notification

Export the PDF from the Health app and make an appointment with a healthcare professional. Bring the report, but also describe what prompted your concern. Useful details include repeated loud snoring, witnessed pauses in breathing, waking up gasping, daytime sleepiness, morning headaches, insomnia symptoms, and relevant medicines or health conditions 9.

Diagnosis requires a comprehensive sleep evaluation plus an appropriate sleep test. The American Academy of Sleep Medicine identifies polysomnography as the standard diagnostic test. A technically adequate home sleep apnea test is also an option for certain uncomplicated adults with symptoms that suggest a higher risk of moderate-to-severe obstructive sleep apnea 3.

A home test is not the right choice for everyone. The AASM recommends in-lab polysomnography instead when certain complicating factors are present, including significant cardiorespiratory disease, possible respiratory muscle weakness, suspected hypoventilation, chronic opioid use, a history of stroke, or severe insomnia. If a home sleep apnea test is negative, inconclusive, or technically inadequate while suspicion remains, the guideline recommends polysomnography rather than assuming sleep apnea is absent 3.

Your clinician can use the history and test results to determine whether sleep apnea is present, which type it is, how severe it is, and whether treatment is appropriate. The Apple Watch report can start that conversation, but it cannot make those decisions.

What if you have symptoms but no notification?

Talk with a healthcare professional even if the watch has remained silent. Common reasons to seek evaluation include breathing that repeatedly stops and starts, frequent loud snoring, gasping during sleep, and persistent daytime sleepiness or fatigue. Some people do not recognize their nighttime symptoms until a partner notices them 9.

Also check whether the feature is fully set up and whether the Health app has enough usable nights. Repeated No Data entries may point to fit, battery, motion, storage, or sleep-tracking problems. Correcting a collection problem can improve the chance of completing an evaluation window, but it does not make the watch a diagnostic test 5.

The bottom line

Apple Watch can detect a multi-night wrist-motion pattern that may be associated with moderate-to-severe sleep apnea and notify an eligible user. That is useful as an early warning, particularly because the check happens in the background over time. The tradeoff is that the feature misses some people, does not measure diagnostic AHI, and cannot confirm or exclude sleep apnea.

Treat a notification as a reason to get evaluated, not a diagnosis. Treat persistent symptoms the same way even when no notification appears.
